Training week.
Pre-Test, post-test and Evaluation forms
The pre-tests, post-tests and evaluation forms are for those completing the training June 22-26, 2009. We are not using identifiers on the tests and will average the results.
Pre-Test
Prior to entering the CDM MRC Training Site, please complete the pre-test questions. You will need to download the pre-test onto your computer, type in the answers, save it, and then return it as an attachment to kjuul@salud.unm.edu.

Post-Test
Upon completion of the training, please follow the instructions above to complete the post-test. Please submit the post-tests no later than Jun 27, 2009. You can download the post-test here: POST-TEST
Evaluation
To help us evaluate the training, we would greatly appreciate your feedback. If you had difficulty accessing Second Life and were not able to complete the training, we would like to hear from you as well. You will need to download the evaluation form, save it to your computer, type in your comments, save, and then send the evaluation form as an attachment via email to unitcord@cdmmrc.org.
